Question: Are there really 12 zodiac signs, as we know them from the western world? How much of the public knowledge about them is true?

If you mean by your question, the constellations in the sky, yes there are 12 constellations in the sky. There are many others as well, but 12 main ones that we see in our night sky. Look for example, the 13th constellation or forgotten constellation as they call it, Ophiuchus, or the Serpent Bearer.
